Intake and consent forms

Build your own intake, consent, and health update forms, choose who gets them and how often, and see on each client which are done.

Your forms

Open Settings → Forms. Owners, admins, and managers build and change forms; the front desk sees the list.

Every clinic starts with the built-in Visit intake: preferred pressure, an area to skip, health flags, and the cancellation policy, with a signature. Its questions stay fixed because the visit note and the chart's alert bar read them. Choose Duplicate to make your own version.

To start from something ready, pick a template under Start from a template:

  • Visit intake
  • Massage intake
  • Chiropractic intake
  • Consent to care
  • Draping and sensitive areas

The consent templates are plain-language starting points, not legal advice. Review the wording with your own advisor and change it to fit your practice before you turn them on.

Building a form

Give the form a title and a kind (intake, consent, health update, or other), then add questions:

  • Short answer and long answer
  • Yes or no
  • Pick one and pick any
  • Date
  • Number or scale, such as pain from 0 to 10
  • Section heading or instructions, for consent text
  • An I agree checkbox
  • Signature

Each question has a label, an optional help line, and Required. Show only when hides a question until an earlier yes/no or choice question has a certain answer, for example "Which area?" only when someone answers yes to "Is there an area to skip?". Drag questions, or use the arrows, to change the order.

Fills the visit note links a question to the note: preferred pressure, an area to skip, pain today, how they are today, and health flags start Subjective and the alert bar the same way the built-in intake does.

Changing the questions makes a new version. Answers a client already gave stay with the version they filled in.

When a form goes out

Turn on Send with bookings, then choose:

  • Who gets it: everyone, or new clients only
  • Which services: all, or only the ones you tick
  • How often: every visit, once, or every few months

When a client books, they get one text or email with a link. They fill in each form they owe, one after another, on their phone, and sign with a finger. The link never names the service. Forms also wait in the client hub under Forms until they are done.

Seeing who is done

Visit cards on the schedule show Forms when everything for that visit is done, or how many forms are still due.

On a client's profile, the Forms tab lists every form with its date, version, and whether it was signed. The front desk sees which are done and can Send a form at any time; it goes with the client's next visit. The client's practitioners, owners, admins, and managers can Open a form to read it or download a PDF. Every look is recorded in the audit log.

Answers and signatures are encrypted. Archiving a form stops it going out; past answers stay on each client.
